Work Permit Applications
Please download and fill out the appropriate form and e-mail it to Maureen Brown.
When emailing Ms. Brown, please include the following information:
- Photo ID (License or School ID)
- your home address
- where you were born
If a student is unable to download and email the necessary document to the EPS,
it may be mailed to 121 Vine Street, Everett, MA 02149.
Please include a picture of an ID (this may be a school ID), or birth certificate.
