Health Insurance Opt Out Program

It’s that time of year again when we open up the Health Insurance Opt Out Program for City Employees.  Please review this important information. Here are a few things to keep in mind:

  • Forms must be submitted no later than Thursday, November 30th in order to receive the buyout.
  • All corresponding documentation must be attached or the request may be denied.
  • Forms must be completed in full.             
  • If you are requesting an opt out, you must provide a letter from your provider stating that as of October or November of 2023 from your provider that you are covered in a health insurance plan.
  • Please note that if you submit your forms after November 30th you opt out and may be denied.
