Insurance Opt Out
The Health Insurance Opt-Out Program is now open! Attached to this message are the program guidelines and the required waiver form. Please review this important information.A few things to keep in mind:
- The Human Resources Department will not be accepting documentation sent by email or in person.
- All appropriate documentation must be included with the waiver form by the deadline, or the request will be denied.
- Forms must be completed in full. If they are not, the payment will be delayed or the request will not be processed if submitted after the May 16th deadline.
- When submitting the form, you must provide a letter or documentation from the health insurance carrier stating that you have active coverage in April/May 2025. This documentation must also provide the primary policyholder’s name.
- Providing an insurance ID card is NOT sufficient documentation.
- If you submit your forms after May 16, 2025, your opt will NOT be processed.
- As a reminder, all documentation must be submitted electronically through this portal.
