This form should be used to submit CORI requests for volunteers or vendors. An Everett Public Schools employee must verify the original ID of the applicant in person and complete the bottom of the form. Once the form is complete, it should be submitted here by an EPS employee using this electronic form. If you need to find the CORI form, use this link.
