This form should be used to submit documentation for the new FSA and Teamsters benefits selected in the 2026 open enrollment period
(April 27, 2026 - May 8, 2026)
All existing FSA participants must re-enroll through the FSA portal. You can use this link to do so: https://getebm.com/spending-accounts/
No other forms will need to be submitted. All selections will be completed with a Benefits Advisor.
Remember to schedule a one-on-one meeting with a benefits counselor during the enrollment period. These sessions are the best way to ensure you fully understand your options and make the most informed decisions for yourself and your family. Take the time to make the best decisions for you and your family during this important period.
