Registration Requirements

Registration for all grades takes place at the Parent Information Center.
The student must reside in the City of Everett in order to attend the Everett Public Schools.
The following documents must be provided at the time of registration:
PROOF OF BIRTH DATE (One of the following documents)
• Birth Certificate — Original or certified copy. (If born in the U.S., you must provide a birth certificate)
• Passport
• I-94 Card
• Resident Alien Card
 
PHYSICAL EXAMINATION (Within the last 12 months)
 
STUDENT HEALTH INSURANCE (Student health insurance card or healthcare provider later)
 
VERIFICATION OF RESIDENCY (Must submit ONE DOCUMENT from EACH of the following three categories)
 
EVIDENCE OF RESIDENCY
EVIDENCE OF OCCUPANCY
EVIDENCE OF IDENTIFICATION
• Recent mortgage payment or property tax bill, or
 
• Copy of lease and recent rental receipt, or
 
• Residency certification (provided by City Clerk's Office and recent rental receipt, or
 
• Section 8 or Housing Agreement
• One of the following bills (dated within the past 30 days) or evidence of utility account established at an Everett address with name:
• Excise Tax
• Gas
• Electric
• Home Telephone (NO cellphone)
• Cable
 
A valid original of one of the following
• Massachusetts Driver's License
• Massachusetts Photo ID
• Passport/Green Card
 
 
 
 
 
IMMUNIZATION RECORD (Vaccinations must be up to date)
 
IF AVAILABLE OR APPLICABLE
• Previous school transfer card or withdrawal form
• Individualized Education Program (IEP)
• Complete school record, including discipline record
 
ACCESS TEST RESULTS (For ELL students, if applicable)
Out-of-state English Language test results are acceptable
 
MCAS RESULTS (For students in Grades 3-12)
 
Registration Takes Place by Appointment Only at the Parent Information Center, which is located on the lower level of Everett City Hall (Church Street Entrance)
